What you need to know

Get your annual preventive exam from the comfort of your home through Catapult Health VirtualCheckup. This is a convenient option if you can’t get to your primary care physician (PCP) for your annual preventive exam. Also, completing a VirtualCheckup before October 31 can help you earn a wellness contribution to your Health Savings Account (HSA).

Who’s eligible

Employees, spouses and dependents enrolled in the Simplot Medical Plan VirtualCheckup.

How it works

You’ll register for your VirtualCheckup from your computer or mobile device. Next, you’ll complete a blood test and blood pressure check. Finally, you’ll review your test results with a nurse practitioner via video visit and receive a personalized action plan. It’s that easy.

Each VirtualCheckup includes:

Height
Weight
BMI
Abdominal circumference
Blood pressure
Heart rate
Total cholesterol

HDL
LDL
Triglycerides
Glucose and/or A1c
COVID-19 assessment
Depression screening
Medication review

Earning a wellness contribution

Completing your VirtualCheckup by October 31 satisfies one of the two activities for earning your HSA wellness contribution. Keep in mind that it can take up to 30 days to complete. If your checkup is requested after October 1, it may not be completed by the October 31 deadline.
