Visit mavenclinic.com/join/simplot or download the Maven Clinic app to enroll. Employees need date or birth and legal first and last name as listed in MySimplot. Dependents enroll using first and last names on file with Blue Cross of Idaho.
Maven is available to all U.S. employees and dependents enrolled in the Simplot Medical Program at no additional cost.
