How to Enroll
What you need to know
Are you new to Simplot? Be on the lookout for your benefits enrollment packet that will arrive in your mail with special enrollment instructions for completing your new hire enrollment. Watch the 2025 benefits presentation (in English or in Spanish) and check out helpful tips to help you with your new Simplot benefits. You must enroll within 30 days of your hire or change in status event to have Simplot benefits. If you miss your enrollment deadline, you will not be able to enroll until the next Annual Enrollment period or if you experience a qualified change in status event.
How it works
If this is your first time covering a dependent in your Simplot benefits, you will need to provide documentation of their relationship to you within 75 days from the date of your event. This includes a copy of your marriage certificate and each child’s birth certificate. Your dependents will not be covered without this documentation.
Know when to enroll
You must enroll within 30 days of your hire or life event, or during Annual Enrollment each year. Please refer to your enrollment packet for your enrollment deadline.

In addition to yourself, you may be eligible to enroll certain family members in your Simplot benefits.

Decide how much to contribute to your HSA
Remember, you can always change your contribution amount anytime throughout the year.
Enroll online or by phone
To enroll, go to employee.simplot.com (Click the Empyrean Benefits Enrollment tile) or call 800-254-3252.
Declare your tobacco status
This secures your eligibility for the Simplot Medical Program discount rate. You must make this declaration every year or you’ll default to the standard rate. NOTE: Each time you enter the enrollment system, you will be asked to declare your tobacco status. You must confirm your enrollment before exiting the system or you will be defaulted back to the standard rate.
Print your confirmation statement
Confirm your elections and print your Confirmation Statement. Note: If you don’t have the option to print your Confirmation Statement, you have NOT completed enrollment and your entries will not be processed.
What happens if I don’t enroll by the deadline?
If enrollment is not completed within 30 days inclusive of the date you became eligible, you will not be allowed to enroll for coverage until the next Annual Enrollment period, unless you experience a qualified change in status (like getting married or divorced, giving birth or adopting).
Here’s how it applies to your coverage:
- You won’t be enrolled in the Dependent Care Reimbursement Account.
- You won’t have medical, dental, and/or vision coverage.
- You won’t be eligible for the discounted rate on the Simplot Medical Program for being tobacco- and vape-free.
- You won’t have HSA payroll deductions.
Print your Confirmation Statement
Keep your Confirmation Statement with your important documents for your records. NOTE: If the system doesn’t offer you a Confirmation Statement to print, you have NOT completed enrollment.
