Eligibility
What you need to know
Most employees are eligible to participate in the J.R. Simplot Company Group Health & Welfare Plan, and Simplot does not require you to work any minimum amount of hours in order to be, or remain, eligible for benefits. If eligible, coverage is available the first of the month following your hire date. If you have questions about your benefits eligibility, contact the Simplot Benefits Service Center at 800-254-3252.
Who you can cover
Generally, if you’re eligible for Simplot benefits, you can also enroll certain family members.
How it works
If this is your first time covering a dependent in your Simplot benefits, you will need to provide documentation of their relationship to you within 75 days from the date of your event. This includes a copy of your marriage certificate and each child’s birth certificate. Your dependents will not be covered without this documentation.
Social Security numbers
You’ll be asked to enter your dependents’ Social Security numbers when you enroll them, so we encourage you to have them on hand. If you don’t have them on hand when you go to enroll, it’s okay — you are able to enter them at a later date.
