Life and AD&D
What you need to know
Life and accidental death and dismemberment (AD&D) coverage helps you financially prepare for life’s serious situations. Life insurance pays a benefit to your beneficiary, such as your spouse, kids, friend — anyone you choose — in the event of your passing. AD&D insurance pays a benefit to your beneficiary in the event of your accidental death, and it’s in addition to your life insurance benefit. AD&D insurance also pays a partial benefit to you if you become seriously injured or lose a limb.
Who’s eligible
Most employees are eligible to enroll in Life and AD&D coverage. If you’re eligible, you can also enroll certain family members.
How it works
Basic Life Insurance
If you are eligible, Simplot automatically provides this benefit to you at no cost! The coverage amount is 2x your Annual Earnings.
Basic AD&D Insurance
If you are eligible, Simplot automatically provides this benefit to you at no cost! The coverage amount is equal to your basic life insurance benefit (2x your Annual Earnings).
Voluntary Life Insurance
If you would like to increase your coverage, you can buy Voluntary Life and AD&D coverage for yourself and/or your spouse or children. You pay the full cost of any Voluntary Life and/or AD&D Insurance you choose, and you receive a group discount on your rates. The coverage is also portable and/or convertible, which means you may be able to take it with you if you leave the Company. If you’re new to Simplot or newly eligible for the voluntary program, you are guaranteed a certain level of coverage if you enroll within 31 days of becoming eligible. Your cost for Voluntary Life and AD&D Insurance depends on your age, your base salary (as of January 1) and tobacco use.
Designate a beneficiary
It is important to have a beneficiary designated for your Life and AD&D Insurance so your benefits will be paid out according to your wishes in the event of your passing. You can name one person or several people as beneficiaries. You can even name different beneficiaries for your Basic and Voluntary Life and AD&D coverage. And you can change your benficiary(ies) at any time through the benefits enrollment system by going to employee.simplot.com and clicking on the Emyprean Benefits Enrollment tile.
Evidence of Insurability (EOI)
EOI is proof of good health. It is important to enroll within your 31-day eligibility window. You will be required to provide EOI in order to increase your or your spouse’s coverage, or to enroll in any coverage outside the initial 31-day eligibility window. Your request to enroll later or add to your coverage will be either approved or denied based on insurance company underwriting guidelines.
